Common things that cause USB issues:

Lack of power for the requirements of the hardware (Use a powered USB hub or direct connect to the PC. Or add a second power line to the USB supply)

Improper soldering (follow best practices)

Poor connections (maybe damaged)

Too many connections in the USB line from PC to individual hardware piece (data corruption - less is best, solder will be better than a pin)

Incorrect wire placement (crossed wires) - USB and Aviation Wiring

Cables are too long (data degradation - 1.5m or less would be best. 2m might be ok..)

Bad cables (not all USB cables are the same, some only deliver power etc..)

Bandwidth - Can also be an issue if you have a screen attached in particular. Minimize the number of high bandwidth devices on the same USB controller.
