General description
The Resident Assessment Instrument-Minimum Data Set (RAI-MDS) was established by the Centers for Medicare and Medicaid Services (CMS) to comply with nursing home-related policies. The VA uses this standard assessment to ensure consistency with national nursing home standards, meeting standards, and facilitating comparisons among Community Living Centers (CLCs), State Veterans’ Home (SVH), and nursing homes in the community. It includes information on diagnoses for conditions that may affect activities of daily living, cognitive status, mood and behavior status, medical treatments, monitoring, and mortality risk.

Limitations
End of life symptoms for nursing home residents, such as bleeding or having a dry mouth, and inappropriate practices, such as not adhering to advanced directives, are not included in the RAI-MDS.
