General description

CDC's Community Health Status Indicators (CHSI) provides public health profiles for all 3,143 counties in the United States. Each profile comprises key indicators of health outcomes and factors that influence health outcomes, such as social determinants of health (SDOH) - access to and quality of health care, health behaviors, social factors, and the physical environment.

CHSI data may be used by organizations conducting community health assessments to determine the health status and disparities of the community; promote a shared understanding of the numerous factors that can influence an individual's health; and improve population health through the mobilization of collaboration within multisector partnerships.
