After you have downloaded and extracted the scoreboard, open OBS:

From the "Docks" menu then click on "Custom Browser Docks..."

Name the Dock and type the location of the "control_panel.html" file you downloaded.



Result should be:

Drag the dock to the OBS UI to dock. (if it won't dock make sure the UI isn't locked in the "Docks" Menu)

Select the scene you want to display the scoreboard, in the "Sources" panel click the "+" in the lower left. Choose "Browser" from the list.



Name the source:

Configure the browser source. Type the location of the "browser_source.html" file that you downloaded.
Make sure that the Width is 1920 and the Height is 1080.
(This must be set to 1920x1080 regardless of your stream settings.)
*Do NOT check the "Local File" checkbox*
