Which affiliation to use?

Usually, we use following affiliation: "Department of neurosurgery, School for Mental Health and Neuroscience, Maastricht University". For internal registration you can often just use MHeNs, DIV 3.

If you have a joint position with another school or faculty, your affiliation may differ. Discuss with your supervisors either way.

In successfactors you can find a organogram as well, indicating your affiliation.
