The Skinwalker can only spawn in Level 0 of the Backrooms and requires multiple players to be on the server. Additionally, it will only appear after three intercom events have occurred. Once these conditions are met, the Skinwalker will spawn in the world.

When first spawning, the Skinwalker selects a random target and immediately takes control of them. The player will from then on only be able to spectate the skinwalker not being able to do anything. The following effects apply to the targeted player:
- A jumpscare screen appears.
- The player loses control of their character.
- Commands, chat functionality, and movement are all disabled.
If the player attempts to move, text displaying "You can't move anything. This isn't your body anymore" will display in chat and if the player attempts to type in chat or run a command, text displaying "No one can hear you scream" will display in chat. - Before it spawns, the game secretly records short voice clips of players speaking, which the entity later uses for deception.
- They will be unable to leave the game. The "Back to the title screen" button will be replaced with "There is no Escape"
While passive, the Skinwalker mimics the appearance of its target, using their exact player skin. It also employs the recorded voice clips and crouching mechanics to imitate normal player behavior, attempting to deceive others. The deception mechanics include:
- Playing recorded voice clips of the target.
- Crouching to mimic real players.
- Wiggling side-to-side to appear natural.
- Looking around randomly for short periods to simulate curiosity.
- Punching the air as if interacting with the environment.
However, the more it is interacted with, observed, or remains near players, the more its Suspicion Meter increases. When this meter is full, it enters its next phase.

Once fully suspicious, the Skinwalker transitions into its transformative state, indicated by:
- A cracking sound as it breaks its neck, with its head tilting slightly and looking over its left shoulder.
- A loud screeching noise to signal its transformation.
- All lights around it flicker, creating an eerie and unsettling atmosphere.
- A brief delay before it becomes fully hostile.

In its hostile form, the Skinwalker takes on a nightmarish appearance:
- Retains the targeted player’s skin, but with black tentacles sprouting from its back.
- Its head turns entirely black, with two glowing white pupils being its only facial features.
- Moves using its tentacles, which produce a distinct thudding sound, audible through multiple layers of walls.
- Periodically emits clicking noises from its mouth.
- Plays distorted voice clips of the player it's mimicking in a lower pitch. Different distortions can be stutter, bitcrush, and reverse.
Once it spots a player, it unleashes a horrific scream and initiates its chase. From this moment, a chase timer starts. If the Skinwalker fails to catch its prey before the timer expires, it despawns, granting the players temporary safety until another instance spawns.
- In its passive state, the Skinwalker has 1000 hit points (2.5× the Warden's HP) and can be killed if enough damage is dealt before transformation.
- Once it enters transforming, hostile, or chasing states, it becomes completely invulnerable and cannot be harmed.
- The Skinwalker relies on deception, making communication between players crucial for survival.
- The recorded voice clips and imitated behavior can make it difficult to distinguish from real players.
- Avoid excessive interactions with suspicious entities to delay its transformation.
- If a player runs the
/skinwalker <target>
command on themselves in singleplayer, they will be forced to spectate the Skinwalker with no way to escape.
The/release <target>
command will not work, nor will the player be able to leave the game normally.
The only known solution is to leave the game and never enter the world again, as rejoining will result in being permanently trapped in spectation mode. - If players have already survived an encounter with a Skinwalker once before and encounter a new one in its passive state, its head will be at a 90-degree angle, resting on its shoulder.
The Smiler bears the appearance of a wicked grin and two glowing eyes and only spawns during the blackout event on Level 1.
- While in the Smiler's line of sight, players will experience a static overlay on their screen and begin to take continuous damage.
- Smilers are invulnerable—they cannot be harmed by any means.
- The only ways to escape a Smiler are:
- Breaking line of sight and waiting for the lights to turn back on.
- Running at them with a flashlight turned on, which forces them to disappear.
- Smilers only spawn in Level 1 of the Backrooms or in the
spb-revamped:level1
dimension during the blackout event. - Attempting to spawn them elsewhere using
/summon spb-revamped:smiler
will result in nothing happening.
