SimplePhpFormBuilder is a Aplication that based on a jQuery.formbuilder, PHP and MySql database , allowing you to build and management simple html forms. I also use PHPMailer library To handle sending emails, Tracy library for debug mode and ApPHP EasyInstaller for installation process.
