The New York City Department of Health and Mental Hygiene (DOHMH) maintains health center boundaries. These are also known as "Health Center Service Areas." Health centers are used for reporting health statistics. Health-Areas nest within Health-Centers.

Older systems sometimes refer to these as Health Center Districts.
