This board is the not often discussed very cost reduced version of the IBM model M. As is common on these boards, the controller did not function. There's some capacitors that are usually the cause and need to be replaced. This requires the board to be fully disassembled so I went ahead and did a full cleaning while I was at it. The caps were indeed the issue and the board is now working great.
