mysql schema

In comparison to classic PACS DB, this schema:

• concatenates both patient and study attributes in an unique table (E as exam or estudio)
• stores encapsulated objects (cda, pdf), gsps, kos, sr, ... at series level, table S (as series)
• registers stacks, GSPS and KOS as groups of frames related to the series in the G table (as group)
