Description

The System Pathways Report includes the pathways of clients served by a homeless system. This report can be used to understand:

  • The flow of clients served through homeless projects in the CoC, including where clients are entering from and exiting to.
  • The demographic breakdown of clients served by the homeless system (race, veteran status, disabling condition, household chronic at entry, or participated in CE.)
  • The average days clients are enrolled in a project.

Universe

The System Pathways Report uses the Additive Universe Selection.

Results

1. Live-Filter Report results can be filtered by Race, Veteran Status, Household Type, Head of Household Only, and CE Involved. CE Involved limits the universe to clients who were enrolled in CE.

2. Sankey Chart The Sankey Chart displays clients with open homeless enrollments in the selected universe. System Pathways are limited to those that proceed through the continuum. If a client goes backwards within the continuum (from more housed to less housed) the regression is ignored for the purposes of this report unless they first exited to a permanent destination. Clients who exited to a permanent destination and then subsequently enrolled in a homeless project are counted in the Returns to Homelessness category. The progression of least to most housed is as follows:

  • Street Outreach (SO)
  • Emergency Shelter (ES)
  • Safe Haven (SH)
  • Transitional Housing (TH)
  • PH - Rapid Rehousing (RRH)
  • PH - Permanent Supportive Housing (PSH), PH - Permanent Housing (PH), and PH - Other Permanent Housing (OPH)

Each pathway through the system displays the percentage of Entering From and Exiting To. Hovering over pathways also displays the percentages and client count for each pathway. Clicking on a specific pathway (e.g. SO to PH-PSH) opens a page with details of clients for that pathway.

3. Results Table Pathway information is also displayed as tables below the Sankey Chart. The results are divided by project type and include the Clients, Average Days Pre-Move-In, and Average Days Post-Move-In. The tables include the Count and Percentage for clients Entering From and Exiting To.

Clicking on the Client Details button opens a page with details of clients for that project type. Details can also be viewed by clicking on the hyperlinks in the Count column of the table.

4. Demographic Breakdown by Clients and Average Days Results can be further analyzed in the Client and Average Days charts. The charts include: Served by Homeless System, Project type, Destination (Client section only), Pre-Move in (Average Days section only) and Returns to Homelessness.

The Demographic Breakdown dropdown can be used to filter the results on the associated bar chart and table. Breakdowns include: Race, Veteran Status, Disabling Condition, Household Chronic at Entry, and Participation in CE. The bar chart will only display results for categories where the client count is greater than one.

5. Client Detail Client details are available by clicking the blue hyperlinks within the report. Details include: Personal IDs, First Name, Last Name, Race, Disabling Condition, Veteran Status, Stay Length, Days Before Move-In, Days to Return, Household Chronic at Entry, Participated in Coordinated Entry, and Coordinated Entry Assessment.

6. Download The report can be downloaded by clicking on the Download button at the top of the report. The report downloads as a PDF of the Sankey Chart and associated table result by project type.

Calculation Notes

  • Uses an ordered set of project types to express a client's journey through the continuum. This is loosely based on the HUD tool STELLA.
