Click 'Medical Record Request' to access the form above. A message above will appear and it's recommended to read it.

• To print the medical record request:

1. Click 'Open Medical Release Form' to open a medical release form.
2. Fill out the text boxes with the information required.
3. Click 'Print' to print out the medical release form.

• Click 'Cancel' to go back to the main menu
