To get to the insurance information page:

1. Click 'About me' to expand the selection in the main menu
2. Click 'Insurance Information'

To enter your insurance information, you have to:

1. Enter the insurance name in the "Insurance" box.
2. Enter the insurance number of your insurance in the "Insurance Number" box.
3. Enter the insurance's member ID in the "Member ID" box.
4. Enter your insurance's group ID in the "Group ID" box.
5. Enter your insurance policyholder in the "Policy Holder" box.
