Usually these errors mean that you have broken BD (BlueRay Drive)
SW Method
Those stuck on Jailbroken FWs (4.55 / 5.03 / 5.05+ / 5.5x / 6.xx / 6.20 / 6.72 / 7.xx) could also use software method:

HW Method A - reusable
Works almost with all models (MTK is not confirmed)

Find a working phat 12xx console (master)

Unsolder Renesas chip (RC1) from it
-
Unsolder dead Renesas chip (RC2) from your "NoBD" console
-
Now install the working Renesas chip (RC1) to your "NoBD" console
-
Assemble and update your "noBD" console via recovery. Console should be updated now without any issue

Disassemble your "noBD" console again and unsolder Renesas chip (RC1)

Return Renesas chip (RC1) back to master console
-
Update master console to same FW (that it has) via recovery. This will "reset" Renesas chip (RC1) to be useable again
Notes:

Was tested with master console 1200 JP region

Master console had 11.00 FW
HW Method B - one time
Works with phat 10xx, 11xx and PRO models only

Find a working drive daughterboard from any phat 1000, 1100 or PRO (e.g. from dead console) with FW lower than your current one.

Unsolder Renesas chip from it and keep it.
-
Unsolder your dead drive Renesas chip from drive’s daughter board.
-
Solder the working Renesas chip on place of your original one.

Install drive daughter board and install the drive to your console and update it either from dashboard or recovery.

Console should be updated now without any issue.

If you want to re-update it again you should repeat this steps.
Notes:
-
Donor chip should come from SAC-001 board.
-
It is better if you could find one with Renesas chip that made in Japan.

Because this chip have a special bootloader that help in this trick.
PSN games unlimited copy

Add PSN accounts with games to your console and activate it as a primary console and download all games that you want from this accounts.

Power off your console and dump your SPI flash and Syscon 2 or 3 times to be sure that you have a correct dumps.

Remove the current HDD and install new one and reinstall the same firmware that you had again.

Add same PSN accounts that you had added in first step and deactivate this console from all accounts now.

Power off your console again and restore your SPI Flash and Syscon dumps that you had made in step 2.

Replace the new HDD with your original one (very important).

Power on console again and now you have activated PSN games
