1. Create developer account and pay $99
2. Click here
3. Create development and production certificates
4. Click here
5. In here add new app (and configure it). You may add extra features in a future
6. Apply (click) on each certificates (development.cer/distribution.cer)
7. Add apple ID to your Xcode (Preferences - plus button - apple ID
8. Type name of your application (from appstoreconnect.apple.com/apps) and choose account to apple ID
9. Connect any iOS device to computer and click on refresh button to send device info to developer.apple.com (provision profiles will be created)
10. Prepare your build and check it. Upload to appstoreconnect. Add tester users
11. It's all.

Mark 1 If you choose iCloud in capabilites change version (build) of your application and finally you will see icloud directory in your iCloud
