Part 1: Character Setup, How to Leave the Capital, & How to beat the Game
Character Creation
Make sure to set up your character to an optimal build you like. Tag skills, perks, and skill points matter! You will only receive 8-12 skill points per level dependent on intelligence and leveling is SLOW. In addition you will not receive double skill points on your tag skills. If you do not know what the special skills or the skills affect I would suggest reading carefully through each as you go through character creation.
Traveling to the Mojave
Follow the New Canaan Questline to Get to the Train Station, then select the TTW quest when it pops up. It is HEAVILY recommended you play through Fallout 3 first and take things slowly.
Beating FO3 & Playing Post-Game
When you are about to beat Fallout 3, make sure to send Sarah Lyons or a companion inside as well! Otherwise the game will end and you will not continue into the DLC and be able to play post end game
Part 2: Gameplay Tweaks & Controls
Tweaks
Gameplay tweaks as well as mod configuration settings are extremely important for tailoring the game to how you would like to play.* In addition, they can help fix overlaps between multiple mod/in game key binds. Please refer to the list below to change the proper key binds to help minimize this overlap. Below these key bind changes will also be a full compilation of tweaks and what they do alongside our very own @Kragos#2049 tweaks list.

Tweaks & What They Do
- 2 Hotkey Restored - replaces the 'Ammo Swap' icon with a functional weapon slot '2' (rebinding the Ammo Swap Key may be required)
- Activating While Seated Doesn't Stand Up - prevents standing up when you press 'Activate' without a ref selected while seated
- Aiming Unholsters Weapons - take out your weapon when aiming
- Allow Activate While Animation Plays - allow activating objects while firing, reloading, jumping etc.
- Allow Sleep Wait With Reduced Max Health - allow sleeping while max health is temporarily reduced
- Allow Weapon Hotkeys While Equipping - use weapon hotkeys while switching weapons
- Better AutoWalk - moving left/right does not cancel autowalk and add backwards movement support
- Better Pickup Prompt - show the value and weight for a whole stack of items while looting and indicate the weight in red if it would overencumber you
- Billy Goat Mode (Climb Steeper Angles) - allow adjusting of max walking/jumping/autowalk climbing angle

Disable Show Quest Location - disable the "Show Location" button functionality in the quests tab

Don't Multiply Item Stats By Stack Size - show the stats of an individual item instead of the entire stack in inventories

Drop Item Hotkey - add a key 'Q' to drop items in the PipBoy

Extended Menu WASD Support - allow WASD for movement and spacebar for interaction in menus. Moving left/right will jump down a page in listboxes. TAB goes back or closes any menu. Scale scroll speed in menus while Shift is held (optional). Navigate the Map Menu by holding alt and using WASD and zoom in and out with W/S if alt isn't held

More Detailed Stats Menu - show the value modifier amount in the (+) and (-) for skills/SPECIAL, and show them above their min/max

Power Armor Prevents Screen Blood - prevent screen blood effects if you are hit on your power armor

Quest Compass Height Indicator - indicate whether a quest marker is above or below the player on the compass

Recurring Challenge Indicator - show (Recurring) beside the progress indicator for recurring challenges

Remove Sneak Label - remove the [HIDDEN] etc. label

Remove Queued Cripple/Critical Messages For Dead NPCs - remove 'Critical Hit'/Crippled Limb' corner messages when the target dies

Right Click Changes To Previous Container Category - make right clicking the title cycle to the previous category in containers/barter/recipe

Agility Scales Movement Speed - movement speed is affected by the agility stat (affects player and all NPCs)

Ammo Count/Type Memory Per Weapon Ref - remember ammo type and count for all player weapons - replaces a vanilla feature where looting an NPC's weapon would also give some ammo

Automatically Unequip Broken Armor - automatically unequip armor when it breaks (affects player and all NPCs)

Cap Skills by SPECIAL - cap the max value for skills in the Levelup Menu based on SPECIAL stats

Charged Attacks Cost AP - require action points for unarmed/melee power attacks

Cookable Grenades - holding a grenade before throwing it decreases its fuse

Custom Hacking Answer Length - customize the hacking word length, min/max, difficulty multiplier and 'Computer Whiz' perk bonus

Detected By Whom - show the names of all actors detecting the player (excluding teammates)

Disarm Requires Weapon Skill - add a chance for mines to not disarm, or instantly explode based on explosion skill

Prevent Reloading Without Empty Clip - only allow reloading if the clip is empty

Quick Use - holding shift and activating the crosshair item will equip it (Aid, Armors and Weapons), or the Book Menu when activating notes (requires Book Menu Restored). Show item stats while shift is held (DAM/DPS for weapons, DT/DR for armors)

Part 3: Survival
After tweaking the game to your liking and determining your character’s origins, it is now time to survive the harsh wastes. A number of mods have made this game much more challenging, but also fair. Unless you are constantly mashing your sprint key, hydration should last you up to 3 days before becoming critically important. In addition, you should be able to last without food for several days without noticing any effects. As for sleep, you can go upwards of a month without it. That being said, your needs still do matter and they will creep up on you before you know it. This is why knowing how to provide for yourself in the harsh wastes is important
Shelter
While the conditions of the Mojave will not make you sick or otherwise harm you (Besides for your Thirst, Hunger, & Sleep), finding shelter is still important for a multitude of reasons.
1.) Your carry weight has been significantly reduced. Finding a place to rest your head and store your gear, whether it be in a tent, mobile RV, or one of the many player homes throughout the wastes is important
2.) Many player homes throughout the wastes host crafting stations for armor and weapon repairs, chem crafting, food preparation, and purification of water. These are all important processes to make sure you are prepared for your journeys.
3.) Healing and assessing player condition is important. The wasteland has been beefed up with many more spawns and much more dangerous enemies. It is best to retreat to your home and find out how to best recover rather than sitting in the middle of the wastes where any manner of man or beast could attack.

Human: Hardcore Needs
Like I said, it may be easy to forget you need to sleep and eat, but hydration is the most rapidly changing stat depending on your movement, the temperature, carry weight/armor weight, and time. While all normal methods of water drinking are still in game, bottling water is one that is new. To do this…
1.) Have an empty water bottle or canteen in your inventory
2.) Approach any source of water whilst crouched
3.) Press your activation key as if you were going to drink from it. The appropriate water will then collect in any and all empty water receptacles on your person
While I will not post every value below, understand that drinking anything other than water such as alcohol and soda will only serve to dehydrate you even more. This also applies to dry foods and other foods. Make sure to read every consumables description before consuming it.
As for sleeping and eating, these are self explanatory. Make sure to watch your meters in your pip-boy and sleep/eat when you begin to feel hungry. All three of these stats will produce negative modifiers once they reach a certain threshold, so making sure your character is healthy will ensure you do not become over encumbered or worse.
Part 4: Medical System
Healing is the essence of life in any Fallout game, but healing has been completely revamped in this mod. New healing items have replaced others, new sickness and diseases have been added, and food/water no longer heals health as it used to. Even worrying about light, moderate, and severe bleeding will be a part of your everyday struggle as you fight through the wastes. Below are all the items you will need to prepare yourself for this/what they do.
Tweaked Healing Items

Blood Pack: Restores 36 health over 12 seconds, damages rads by 24 over 12 seconds, is now marked as medicine to benefit from the medicine skill, and it returns an Empty Blood Pack.

Doctor's Bag: Buffed to restore 54 limb health over 54-18 seconds, and reduces Agility for 54-36 seconds. Fast metabolism grants faster limb healing, but no bonus to healed limb condition.

Auto-Inject Stimpak: Now the Combat Ultra-Stim.
Restores 20 health and 50 sleep deprivation immediately, increases health by 20, Agility and Perception by 1, AP regen by 25%, and attack/movement speed by 10% for 120 seconds, but it reduces your sleep deprivation by 1 each second, for 175 seconds.
Carries the same withdrawal as the Combat Stim, at about twice the likelihood.
Can be diluted into 3 regular Combat Stims, or condensed from 3 Combat Stims and an Empty Syringe.
Has an option for distribution throughout the wastes. Unlike the other items, this setting can be set to 0 and turned off, leaving the stim only in LR locations.
- Auto-Inject Super Stimpak: Now the Ultra-Stimpak:
Buffed to restore twice as much health as the Super Stimpak and increase health by 50 for two minutes, after which your health is reduced by 30 for an additional minute. It also carries a moderate food/hydration penalty, as well as a short sleep deprivation restoration, but a much steeper sleep deprivation penalty over time, all of which take place over 4/5 seconds.
Crafting recipe requires a Super Stimpak, Stimpak, Leather Belt, and an Antivenom.
New Healing Items

Basic Bandage: Heals 15 health over 15 seconds. Heals 10 bonus health over 10 seconds if you're not in combat. Basic, but it won't count toward Overdose, and it's light and relatively easy to find. Bandages will stop BLEED's bleeding effect if you're afflicted by it.

Military Bandage: Heals 25 health over 25 seconds, but it reduces Agility by 1 while it heals. Heals 10 bonus health over 10 seconds if you're not in combat.
Rarer, but better Bandage. Comes with an Agility penalty to account for it's better healing.
- Antiseptic: Grants 10% bonus healing for 45 seconds.
It's more for use in Medkit crafting recipes than individual use. It will apply to food healing.
- Improvised Hemostatic Pill: Grants 25% reduced limb damage for 240 seconds.
Survival 20 or Medicine 20 for visibility + 20 Science to craft.
Requires: Tin Can, Buffout, Healing Powder, Purified Water.
Recipe grants 6 pills. Marked as a food item, so it scales with Survival.
- Solvitol (Painkillers):**Provides +12% DR for 200 seconds, damages dehydration by 15.
Commonly used for Improvised and Survival Medkits. Has no addiction, but it will increase the likelihood of a Med-X addiction if you take it while on
Solvitol. Does not stack with Med-X.

Splint: Grants a small amount of limb healing.

Army Morphine Syrette: Grants 90 seconds of ignored crippled limbs, reduces strength for 120-90 seconds.
Medicine 25 for visibility + 30 Medicine to craft.
Requires: Empty Syringe, Med-X, Surgical Tubing, Purified Water.
Weaker version of Med-X, with a lower addiction chance than Med-X in my Ingestibles mod. Valuable for their weight in resale.
- Combat Stim: Increases AP by 20, Agility by 1, +25% AP regen, and +10% movement speed for 120 seconds, but it increases your sleep deprivation by 55.
Pretty useful. Has a low addiction chance, and a relatively low addiction penalty.
- Improvised Medkit: Restores 12 health over 4-5 seconds, restores 15 limb health over 5-3 seconds, and reduces Endurance and Strength for 45-30 seconds.
Survival 15/Medicine 20 for visibility + Survival 20 to craft.
Requires: Tin Can, Solvitol, Purified Water, Empty Syringe, Lunchbox, Antiseptic. It's not very strong, but it's a relatively cheap way to get your limbs off of crippled for a short time, and it's relatively easy to craft. Marked as a food item, so it benefits from the Survival skill.
- Survival Medkit: Restores 24 health over 8-6 seconds, restores 25-30 limb health over 5-6 seconds, reduces limb damage by 25% for 2 minutes, and reduces Endurance and Strength for 45-30 seconds.
Survival 30/Medicine 20 for visibility + Survival 35 to craft.
Requires: Tin Can, Solvitol x2, Purified Water, Empty Syringe, Lunchbox, Improvised Hemostatic Pill x2.
A moderate improvement on the Improvised Medkit for Survival players. Also marked as a food item.
Small Medkit: Restores 24 health over 4-5 seconds, restores 30 limb health over 10-3 seconds, and reduces Endurance for 40-30 seconds.
Medicine 25 for visibility + Medicine 25 to craft.
Requires: Stimpak, Bandage, Antiseptic, Scalpel.
Not as much direct healing as just using the healing ingredients on their own, but having them all together along with the scalpel, grants you some moderate limb healing.
- Army Medkit: Restores 30-36 health over 5-6 seconds, restores 60 limb health over 10-6 seconds, grants +5 DR for 25-30 seconds, lets you ignore crippled limbs for 60 seconds, and reduces Endurance for 50 seconds.
Medicine 30 for visibility + Medicine 35 to craft.
Requires: Stimpak, Military Bandage, Antiseptic, Scalpel, Splint, Army Morphine Syrettes.
Moderate improvement to healing, substantial improvement to limb healing over the Small Medkit, grants DR and the Med-X effect, but it requires more items, and the Splint means that the Endurance penalty doesn't get reduced by Fast Metabolism.
- Scientific Medkit: Restores 48 health over 12-8 seconds, restores 40 limb health over 20-10 seconds, reduces rads by 30 over 6-5 seconds, and reduces Endurance for 50-40 seconds.
Medicine 40/Science 35 for visibility + Medicine 45 to craft.
Requires: Stimpak, Bandage x2, Antiseptic, Scalpel, RadAway.
More health restoration than the Army Medkit, and it's Endurance penalty is reduced by Fast Metabolism, but the healing is slow, because it uses two bandages.
- Field Medkit: Restores 65 health over 13-5 seconds, restores 60 limb health over 10-6 seconds, grants +5 DR for 30 seconds, lets you ignore crippled limbs for 60 seconds, reduces rads by 36-42 over 6-7 seconds, and reduces Endurance for 75 seconds.
Medicine 50/Science 35 for visibility + Medicine 55 to craft.
Requires: Stimpak x2, Bandage, Antiseptic, Scalpel, RadAway, Preserved Blood Pack, Army Morphine Syrettes, Splint.
Expensive to craft, expensive to buy, but it'll solve all your problems at once, and grants a small statistical bonus to each factor for having them all in such an expensive package.
- Blood Draw Kit: Damages health by 36, reduces Endurance for 3 minutes, and it damages your FOD and H2O by 55, but it grants a Blood Pack, an Empty Syringe, and a Scissors.
Medicine 25 for visibility + Medicine 35 to craft, while holding an empty blood pack.
Requires: Empty Syringe, Surgical Tubing, Scissors, Empty Blood Pack.
Provides the same amount of health damage as a blood pack returns
- Preserved Blood Draw Kit: Medicine 40 for visibility + Medicine 35 to craft, while holding an Empty Blood Pack.
Requires an extra Rad-X on top of the regular Blood Draw Kit.
Just as easy to craft, if you know that you can use a Rad-X for preservation.
Grants a Preserved Blood Pack.
- Preserved Blood Pack: Doesn't deal any rad damage, making it a solid healing item & scales with medicine

Misc Repair Notes

7.) Armor and weapon dismantling can be done at workbenches with the aid of a dismantle kit. This will allow you to destroy your item and receive back components depending on item condition
Part 6: Fast Travel
Fast travel is in the game! But not how you think. Fast travel is one of the best, yet least immersive things about Fallout. However, now that is a thing of the past! There are several ways the player may fast travel which will be detailed below!
Transporters
A faction of roaming wanderers, these people will take you from point A to point B – for a few caps of course. They can be found at any and every settlement and offer 20 travel locations. Why not try them out, find their settlement, or even apply for a PREMIUM travel membership?
Part 7: Drug Synthesizing
All usable chemistry sets (bar a few) have been automatically turned into Drug Synthesizers. These can be use to breakdown chems into compounds. Below is a list of the compounds and their relationship to SPECIAL
