Patients need to perform exercises on their own in physical therapy at home between clinic visits. They are given written instructions, verbal reminders, or generic video URLs to perform. The majority of patients forget exercises when they are not in the clinic. Handouts get misplaced, and instructions do not always translate well into real-life use.

Patients tend to miss clinic appointments or perform exercises with bad posture or technique. When discomfort or pain is felt, patients never know if they should continue or stop. Information is only gathered by patient self-report during follow-up visit. Patient-reported information is incomplete, inaccurate, or inconsistent.

Doctors lack accurate and real-time information about how patients perform their exercises at home. They cannot easily track patient adherence, posture, or progress between clinic visits. Without reliable data, doctors spend time clarifying or repeating instructions instead of focusing on monitoring recovery and making informed treatment decisions.
