After Lama Temple you can save him using reveal and lift, if you ignore him you can't save him later

Clear the way to the ruins in Altin Peak before picking up the Orb of Force in Fuchin Falls

Competently Incompetent (5) 
Pick an imbalanced rowing team and explore.

(You can only obtain this on the boat ride, also you need to discover the cave before going back to the ship!)

Colosso King (10) 
Win the Colosso Finals.

If you lose a battle at the Tournament you miss this, if you have gained enough levels and have good equipment it is not hard though.

Run Isaac Run! (10) 
Outrun the Storm Lizard's tornado and escape Suhalla Desert.

You have to make sure not to kill the Storm Lizard. Before you are able to do this you have to trigger a story event in Suhalla speaking to a soldier. Save in Suhalla before going into the desert to be save.
