The Killer will kill entities where it looks, you have the settings: players (default), items, animals or monsters. To change the setting, place it in the center of the Advanced Crafting Table.
With the Killer-tool, you can change the direction in which it looks in the 4 directions, more above it. Then you will have to place it on the machine while keeping the "sneak" key pressed.
