Red Hazard Card is required for any viral injection surgery

All marked areas from 1-8 must be filled by the surgeon.

If you do not need to take care of your own cages after 72 hour, please check 'DCM' box; Alternatively, if you have any special device or food or liquid, please check the other box.

We usually do not use any chemical hazard in lab, so 'NA' should be marked there.

Please check the viral vector box as needed (AAV in most cases)
