General

A crushing blow is a normal hit or critical hit by an NPC that deals 150% of the damage it would otherwise cause. In patch 3.0.2 the crushing blows mechanics was changed, therefore the following is only relevant for TBC.

miss % + dodge % + parry % + block % (incl. Holy Shield) + (4 * 0.6 %) >= 102.4 %.

Thorough explanation

When facing a mob of equal level as yourself you would need 100 % avoidance. However, only mobs 3 levels higher than yourself is capable of dealing a crushing blow. A mob that is three levels higher than yourself will lower the effectiveness of each avoidance stat by 0.6 %. Therefore you must factor in an additional 0.6 % to each avoidance stat. You have four ways of avoiding an attack because you can dodge, parry or block the attack or the attacker can miss you. Factoring in the level difference: 0.6 % * 4 = 2.4 %

Therefore the required avoidance you need to successfully push crushing blows off the attack table is 102.4 %.
