Found elements with a .glyphicon-* class that were missing the additional required .glyphicon class.

.glyphicon-* classes can't be used on their own; they must always also be accompanied by the .glyphicon class.

Wrong:

Right:

<span class="glyphicon glyphicon-heart"></span>
