Los Caídos Needle Exchange (LCNE, usually said as "lack-knee") is a program, run largely by Licentient volunteers, in both Taqfleck and the city of Los Caídos proper. Like most needle exchanges, LCNE is officially authorized by the state, but there is often pushback from locals who disapprove of needle exchange programs. Jang-mi's main political opponent has attempted to use LCNE as a wedge issue, to some degree of success.

Using LCNE is an anonymous process, although some people choose to share their name and gradually build rapport with LCNE volunteers or volunteer themselves. Needles and other supplies are mostly distributed though pop-up exchanges. Within Licentia, a delivery service is available, but this does not exist for the rest of the city out of concerns of volunteers being ambushed.

Medical professionals from Licentia sometimes volunteer at pop-up exchanges. There is also often someone who works in case management in the local LC government -- such a person is typically not affiliated with Licentia, and can help people manage the resources that LC offers.

LCNE offers on-site C, T, and Z virus testing. People who test positive for these viruses are typically offered a place to stay in Licentia. Some people feel that LCNE is a little too pushy with their offers.
