• Title - form title
• Show in adminhtml menu - if Yes then Form is available from admin menu -> Marketing -> Custom Forms
• Can use as frontend widget - if Yes then form is allowed to be used as widget on frontend
• Form Identifier - for develper usage: Display Widget Form Programmatically
• Group Fields In - select if fields are grouped in fieldsets or separated tabs on form record edit page in admin

For internal use. You can keep here any important information about form, for example what is its purpose or where it is used on frontend.

Here You can manage form fields.

• Field Label - Name of input field
• Frontend Input - Slelect one of Input Types
• Is Required - set if input value is required
• Sort Order - sort fields by this value ascending

• Frontend Form Description - description which can be displayed on frontend widget
• Submit Button Label - replace default label Submit by Your own
