It can be created in a Crafting Table, via shapeless recipe:

Allows you to transfer enchantments from any item, by combining the tome with it in a crafting grid:

Item which enchantments are absorbed by the book remains intact within the grid after crafting, besides having all of it's enchantments removed.

Any effects and data of the original item is preserved when absorbing enchantments from it.

The result of combining the tome with enchanted item is a regular Enchanted Book that has all absorbed enchantments on it, which can be combined with other books or applied to items using Anvil.
