There are no known ways of obtaining this item.

When equipped in the Spellstone slot, it will give the bearer immunity to wide range of environmental damage types and most status effects. It will allow them to freely fly and will render them indestructible. When flying, Heart of Creation will also compensate mining speed penalty, boosting block breaking rate back to regular value.

Flight granted by Heart of Creation is completely similar to the flight in Creative Mode. It also includes ability to accelerate by holding Ctrl and fall damage immunity.

Immortality provided by this relic protects the bearer from any damage types and death causes, preventing their health from getting lower than 1 HP. This includes the Void and /kill command.

Any status effect applied to the bearer will be instantly removed, whether they are buffs or debuffs.

Active ability of Heart of Creation allows player to summon skulls similar to ones shot by Wither. The skulls appear slightly behind the player, scattered within some area above them. After being summoned, each skull will stay in place for half of a second while turning into direction of players sight, being launched afterwards:

Skulls will attempt to target the block player is looking at. If no block is present, they will instead be launched towards the point 64 blocks ahead in the sight direction.

Most of them behave similarly to skulls of Wither and have the same explosion power and radius. There is however a 25% probability to summon stronger version of the skull on each shot. This version is slightly bigger visually, has larger explosion radius and is capable of destroying most solid blocks.

If a skull exists for more than 20 seconds without colliding a block or entity, it will explode right away.
