cmi5 Course Structure - masteryScore (Issue #206)

masteryScore was inconsistently listed as mandatory for the Course Structure. Based on previous PR's and further discussion masteryScore was determined to be optional in the XSD and the description was changed accordingly.

cmi5 Runtime - voiding Statements (Issue #202)

It was further agreed that AU's within the context of cmi5 must not void statements. (as per issue #202) After further discussions it was determined that the best way to achieve this was to require the LMS not to allow the AU access/authorization to void statements. It was agree that the following will be added to the spec:

An LMS MUST NOT grant authorization/credentials to an AU which allows it to void Statements.
