cmi5 spec clarifications

The group agreed to spec clarifications discussed in the last meeting. The following pull request was created as result for final approval at the next meeting.

cmi5 narrative

The group continued to update the LMS course process flow chart and discussed adding "in scope" and "out of scope" talking points for the LMS developer presentation to provide more content for the audience.
