At the first you need to know your Outlook Web Access page URL. Often this address looks like

https://mail-domain-name/owa or https://owa.mail-domain-name/. Pay attention to owa suffix or prefix.

To get you Exchange Server URL, you should replace owa to ews. It should be looks like https://mail-domain-name/ews or https://ews.mail-domain-name/.

After that , just put this values in the correspondent fields at the accounts page form. If you don't have this URLs, you can ask this from your system administrator of your Microsoft Exchange Web Server. Also be patient that OWA notifier work with Microsoft Exchange Web Server 2010 and higher.
