Ability

To use the Ultra Crystal, right-click it when it is in your hand. It will permanently (or until you drink milk) give you a random positive potion effect and use up the Ultra Crystal:

• speed
• haste
• strength
• instant health
• jump boost
• regeneration
• resistance
• fire resistance
• water breathing
• night vision
• health boost
• saturation
• luck
• conduit power
• dolphin's grace

It will apply a random amplifier (from 1-5) to that effect (so you could technically become invincible with Resistance V, though there is only a 1.25% chance of that happening). Additionally, there is a 6.25% chance of instant death (the instant damage potion effect) when using the Ultra Crystal, so use it at your own risk.

Obtainability

The Ultra Crystal is designed to be extremely rare as you have to kill a total of nine Withers to obtain it (five Crystals with one nether star each and four additional nether stars). This is why it may be helpful to have established a Wither farm or something in the End to help you with this task.

Strategy

There is no real advantage to saving an Ultra Crystal for later. Once crafted, you should use it immediately. If you desperately need a different (temporary) potion effect in combat, use the Dark Sword. However, be very wary when using the Dark Sword in conjunction with this crystal. If the Dark Sword gives you a stronger potion effect than you already have, it may override it and eventually remove that effect, so using the Dark Sword's right-click ability is inadvisable.
